NDIS Service Agreement

– Our services are provided in accordance with the NDIS guidelines and regulations.
– A written service agreement will be established between Vital Disability Group and the participant, outlining the supports to be provided, the cost, and the responsibilities of both parties.
– The service agreement will be reviewed regularly to ensure it meets the needs of the participant and complies with NDIS requirements.
– Participants have the right to request changes to their service agreement at any time, and Vital Disability Group will work collaboratively to accommodate these requests where possible.

Privacy and Confidentiality

– Vital Disability Group is committed to protecting the privacy and confidentiality of participants. All personal information collected will be handled in accordance with the Privacy Act 1988 (Cth) and the NDIS Practice Standards.
– Participants’ personal information will only be used for the purpose of providing supports and services and will not be disclosed to third parties without the participant’s consent, except where required by law.

Limitation of Liability

In no event shall Vital Disability Group, nor any of its officers, directors, and employees, be held liable for anything arising out of or in any way connected with your use of this website or our services whether such liability is under contract. Vital Disability Group, including its officers, directors, and employees shall not be held liable for any indirect, consequential, or special liability arising out of or in any way related to your use of this website or our services.

Indemnification

You hereby indemnify to the fullest extent Vital Disability Group from and against any and all liabilities, costs, demands, causes of action, damages, and expenses (including reasonable attorney’s fees) arising out of or in any way related to your breach of any of the provisions of these terms.

Termination

Governing Law & Jurisdiction

These terms will be governed by and interpreted in accordance with the laws of the State of New South Wales, and you submit to the non-exclusive jurisdiction of the state and federal courts located in New South Wales for the resolution of any disputes.
